About
Our Hull and East Riding Assisted Discharge Service supports people to return home safely following discharge from hospital. A medical or social care referral is required.
This service is for people who:
Are medically fit for discharge
Are mobile and able to get in and out of a vehicle unaided
Do not qualify for patient transport
We provide short‑term support for up to 48 hours following discharge, which may include:
Transport home from hospital
A welfare check on arrival home
Practical support to ensure you are safe and comfortable
Signposting and referrals to other relevant services and organisations
The service aims to support a safe transition home and reduce the risk of readmission.
